Transportation Pass

The Franklin County Area Office on Aging has opportunities for residents of Franklin County who qualify. Below you'll find the Gold (FREE) Pass screening form. This form is designed to gather your information to be considered for a transportation pass.

Please note: You will need to supply proof of your income to be qualified for the Gold Pass. In the Gold Pass screening form below, there is a section at the bottom of the form called "Upload Financial Documents". When you click on the "Upload" button, it will open a window where you can select the documents that you want to send to The Franklin County Area Office on Aging. The maximum file size is 250 MB.  

Please note: You will still need to complete a RabbitTransit application. We have provided the link below. Please print the RabbitTransit application and follow the instructions on the form.
